HUD/VASH Case Manager:
The HUD/VASH case manager functions are specifically directed toward homeless veterans and their families or veterans at risk for homelessness and require a senior level clinical social worker. The incumbent is responsible for program development, oversight, monitoring, administration and effectiveness, serving as the primary contact with local Public Housing Agencies (PHAs) and providing case management based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations.
The incumbent works closely with the VA medical center's homeless programs, residential and community care programs, and domiciliary programs to ensure that coordination between these entities is facilitated and veterans direct care, referral, a! nd follow up is continuous and based on a continuum of services. The in! cumbent independently provides clinical psychosocial and case management services at an advanced practice level for eligible veterans and their family members in the program.
The intent of having a clinical social worker who is at the senior level is to allow the work to be done where the veteran resides, therefore 60% or more of the HUD/VASH case manager time will he spent at a community location other than the VAMC or CBOC. The case manager will independently need to promote effectiveness in his or her social work function. Veterans eligible for HUD/VASH come from a wide geographic area. Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving. Services are provided in healthcare settings and in the community. The HUD/VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state and federal agencies.
Senior Socia! l Worker:
Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in the area of specialized social work practice. Senior social workers must be licensed or certified by a state area of specialized social work practice. Senior social workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grand fathered by the state in which they are licensed to practice at the advance practice level. Senior social workers may have certification or other postmasters training from a nationally recognized professional organization or university that includes a defined curriculum/course and internship or equivalent supervised professional experience in a specialty.
The advanced practice social worker can be further differentiated from the independent practice social worker by his/her ability to ! expand clinical knowledge in the profession, provide consultation and g! uidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification. As a Senior Social Worker, employee exercises independent judgment, primarily operating on a day-today basis in an independent manner, maintaining full responsibility to keep the supervisor apprised of activities and any issues in need of close collaboration.
The Senior Social Worker is assigned to administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns. They are also assigned in settings where they have no access to social work supervision, such as community-based outpatient clinics (CBOCs) or satellite outpatient clinics. He/she serves the Rock Springs or Gillette CBOC both in-person as well as via Telehealth. They! may be assigned to work with special patient populations with highly complex health or mental health problems, such VA/DoD liaisons assigned to Military treatment Facilities, or social work case managers in Mental Health Intensive Case Management programs. Although the vast majority of patient population serves discharged and/or retired veterans, he/she has held the responsibility of working with active duty personnel whom are in the process of being discharged. Incumbent is required to work with any patient identified in need or requesting aid as a result of their mental health condition, which are frequently numerous and complex in nature and may be sometimes additionally involve multiple physical complications and/or Traumatic Brain Injury conditions.
Work Schedule : Monday- Friday 7:30am - 4:00pm
