Job Description

Peak Team Transitions Center Case Manager

Position Level:

Team Member

Reports to:

Transitions Supervisor

Degree Requirements:

Bachelor of Social Work, Psychology or related field.

Experience:

Experience working with individuals in need of assistance with negotiating for services or linkage and coordination with other systems.

ROLE SUMMARY

Provide support to clients assigned. Facilitate/link clients to appropriate community service resources. Assist and support clinical services as needed in cooperation with the treatment team.

CORE RESPONSIBILITIES

1. Receives referrals for Case Management services/treatment from therapists; provides initial and on-going assessment of consumer needs for case management, treatment, support and/or skills training.

2. Is familiar with community resources and makes appropriate referrals to those resources.

3. Provides case management monitoring of the Consumer?s condition(s) and progress or lack of progress and communicates this to primary therapist.

4. Advocates on behalf of the Consumer to ensure his/her access to needed resources and ensure their rights are respected.

5. Provides emergency services during work hours and after hours as assigned.

6. Provides psychiatric rehabilitation services to all consumers in need, including: readiness assessment, skill building, individual and group counseling, and case management to improve the Consumer?s functioning and to assist consumer in being successful in his/her treatment goals.

7. Provides comprehensive treatment planning in coordination with primary therapist and Consumer based on Consumer?s goals.

8. Provides liaison services to state and local hospitals as needed.

9. Provides program and case consultation and education services as needed.

10. Is responsible for tracking supported employment activities within Peak Wellness Center and assists with the development, and implementation of supported employment programming as directed by supervisor and Clinic Director.

11. Maintains case records and documentation according to the Policies and Procedures for PWC. Provides documentation reflecting the course of treatment in a timely manner. Expeditiously discharges inactive clients.

12. Actively participates in supervision meeting, clinic meetings, case discussions, planning for treatment provision, and agency development required training activities.

13. Maintains a suitable professional demeanor that includes: contributing to a positive work environment within PWC through the exercise of dependability, flexibility, cooperation, supportiveness and respect for others, communication, creativity, ethical behavior, and appropriate appearance.

14. Completes 55% and 1100 hours per month of case management/therapy services.

15. Maintains therapeutic responsibility for the collection of fees.

16. Other duties as assigned by Supervisor, Clinic Director, or Chief Clinical Officer.

SKILLS, KNOWLEDGE, AND ABILITIES

  • Proficient with MS Office, Outlook and Internet Explorer
  • Excellent organization skills and attention to detail
  • Demonstrates an independent work initiative, sound judgment and attention to detail
  • Ability to handle multiple tasks simultaneously
  • Proficient with standard office equipment

MINIMUM REQUIREMENTS

Bachelor's Degree in Social Work, Counseling or other related human service field. Certification by the State of WY and experience preferred. Willingness and ability to work with an interdisciplinary team.
